Ye NABH ka mandatory precondition hai (Entry Level + full DHSP dono me): application se pehle minimum 3 mahine standards implement karke chalane hote hain. Matlab assessor ko 3 mahine ke LIVE records chahiye:
- registers (autoclave, BMW, NSI, drug stock...) me 3 mahine ki entries
- quality indicators (KPI) ka 3 mahine ka monthly data + trend
- committee meeting minutes
- training attendance + pre/post tests
- internal audit records
Ek aur reality: agar records sirf audit se pehle ke hafte me bane hain to assessor pakad leta hai — ink, dates, handwriting, workflow mismatch sab dikh jata hai. 'Audit-day-only compliance' sabse common chhoti-clinic galti hai. Pre-assessment 3 mahine me ready na ho paye to application cancel bhi ho sakti hai.
