NABH assessor patient ke dental record me kya-kya check karta hai?

IMS.3.a (CREO): har patient ka record complete hona chahiye — UHID, demographics, allergies, history, exam, FDI diagnosis, plan, consent, signed-dated treatment notes, prescription, follow-up date.

Assessor random charts kholke minimum data set check karta hai. IMS-3 SOP ke hisaab se har record me hona chahiye: UHID, naam, age/sex, address, phone, allergies, medical history, exam findings, diagnosis (FDI tooth-chart pe), treatment plan, consent (digital signature), har visit ke treatment notes — signed + dated by dentist, prescriptions, follow-up date. ORALZY ka EMR ye minimum data set enforce karta hai, isliye software me bana har chart by-default pass hota hai. IMS.3.b: har entry authenticated honi chahiye — ORALZY har save pe user-id + timestamp auto-stamp karta hai, aur corrections versioned edit se hote hain (original kabhi delete nahi hota) — assessor ko ye audit trail dikhao. IMS.3.c: structured forms + FDI notation + ICD-10/SNODENT codes. Audit se pehle IMS-7 wali 12-item checklist se apne 30 charts khud audit kar lo.
