SOAP notes — structured progress note

Tooth Chart → "Other notes" tab → "Add note" → Type me "SOAP note" chuno → Note box me Subjective / Objective / Assessment / Plan likho (box me yahi khaka pehle se dikhta hai) → "Save note". Note Smart case sheet ke "CLINICAL NOTES" me chhapta hai.

SOAP

S = patient ne kya bataya, O = aapne kya dekha / naapa, A = assessment / diagnosis, P = plan. Ye ek hi free-text note hai — alag-alag S / O / A / P box nahi hain. Note ke "Voice" button se bol kar bhi likh sakte ho (Chrome / Edge). Aaj ka chief complaint / diagnosis wala record "📋 Visit note" me jaata hai, wo alag hai. Likhna sirf doctor / admin / owner. Mobile: More → Records → Notes → "+" → "New Clinical Note" → "SOAP".

Quick steps

  1. Sections → "Tooth Chart" → "Other notes" tab
  2. "Add note" → Type "SOAP note"
  3. S / O / A / P likho → "Save note"
