Current Medication aur Allergy apne alag column me, ek line par ek entry. Section 1 me Occupation, Referred By, Guardian (minor ke liye), Weight / Height aur Rate card bhi chhapte hain. Gender mandatory hai ("Gender *"); purane bina-gender record par ☐ पु/M ☐ म/F ☐ अ/O box chhapte hain jo haath se tick kar sakte ho. Print: Patient chart → More → Forms → Registration → "📄 Open print form".
